Robert D. Peterson, MD comes to Lake Havasu City with over 30 years experience in practicing Internal Medicine in private practice, hospice, military service, Veteran's Administration and Federally Qualified Health Centers.  He has been Board Certified in Internal Medicine since 1979.


 

Susan T. Peterson, RN has many years experience in clinical andadministrative nursing, and has been a team member in healthcare with Dr. Peterson for decades in private practice and Federally Qualified Health Centers. 

 

Kim McDorman comes to our practice with many years experience in  healthcare.  She is the center of our office services coordination.


Our Certified Medical Assistant brings years of experience in many offices and positions in Lake Havasu and Mohave County. Welcome Tonya Taylor to our team at Your Health Home.

As our Billing Manager, Susan Copeland works behind the scenes to navigate the tortuous and ever-changing tangle of health insurance and government rules and regulations.


We are dedicated to serving the needs of our patients each and every day and plan to establish Lake Havasu City's first private practice Patient Centered Medical Home to provide quality and compassionate care to our community.

PATIENT-CENTERED CARE

NCQA Patient-Centered Medical Home™ standards emphasize enhanced care through patient-clinician partnership

The National Committee for Quality Assurance (NCQA) today announced that Your Health Home, PLLC with Robert D. Peterson, MD of Lake Havasu City, AZ has received NCQA Patient-Centered Medical Home (PCMH) Recognition, Level 3, for using evidence-based, patient-centered processes that focus on highly coordinated care and long-term, participative relationships. This is the first practice in Lake Havasu City to gain this highest level of recognition.

The NCQA Patient-Centered Medical Home is a model of primary care that combines teamwork and information technology to improve care, improve patients’ experience of care and reduce costs.

Medical homes foster ongoing partnerships between patients and their personal clinicians, instead of approaching care as the sum of episodic office visits. Each patient’s care is overseen by clinician-led care teams that coordinate treatment across the health care system. Research shows that medical homes can lead to higher quality and lower costs, and can improve patient and provider reported experiences of care.

“NCQA Patient-Centered Medical Home Recognition raises the bar in defining high-quality care by emphasizing access, health information technology and coordinated care focused on patients,” said NCQA President Margaret E. O’Kane. “Recognition shows that Dr. Peterson has the tools, systems and resources to provide his patients with the right care, at the right time.”

To earn recognition, which is valid for three years, Your Health Home, PLLC demonstrated the ability to meet the program’s key elements, embodying characteristics of the medical home. NCQA standards aligned with the joint principles of the Patient-Centered Medical Home established with the American

College of Physicians, the American Academy of Family Physicians, the American Academy of Pediatrics and the American Osteopathic Association.
